Supports the Healthy at Home/House Calls Program in General Internal Medicine to facilitate the care of disabled patients and allow the program to increase the number of patients served. Funds shall provide operational support, including medical equipment or supplies used by OSUWMC clinicians/providers while providing care to home-bound patients (items are not for distribution to patients). Program is focused on adults with disabilities and chronic illness who are often underserved and living in poverty without access to primary care. Regular visits at home by our primary care doctors and nurse practitioners offers them the same opportunities as other patients for preventative care, disease management, rehabilitation and quality of life.
